Community Partnerships
Communities – of place and of interest – are where people bond, build and broaden possibilities. SLHI invests in the sustainability of healthy and resilient communities throughout Arizona, focusing on the capacity to address local concerns through collective action.
SLHI’s work in communities sought a new level with the recognition of strength-based approaches through Health in a New Key. Working collaboratively in the intervening six years, SLHI and an ever-expanding number of community partners are now engaged and networked through:
- Community Partnership Grants. SLHI is purposefully coalescing community investment around two focus areas: healthy eating and active living (HEAL), and value-based health care (VBHC) – a future state that balances system access, cost and quality effectively. The impetus for this focus comes from a recognition that (1) environmental and systems forces influence healthy living as well as responsive systems of care, and (2) social and systems change strategies require meaningful civic engagement. - Communities of Practice (CoPs). CoPs play an important role in connecting and engaging like-minded individuals and organizations in a generative dialogue. All Community Partnership grantees join the CommunityWorks Community of Practice, where participants challenge each other and SLHI on the matter of how best to trigger meaningful and leverageable impact in their individual communities and in the greater context of Arizona.
- The Technical Assistance Partnership (TAP). SLHI’s collaborative technical assistance efforts are integrated into a number of community partnership efforts, providing wraparound opportunities for leverage and connection.
Putting change into practice at any level is challenging, let alone in communities of various size, definition and shape. Disciplined, focused, integrative strategies provide the best chance for success, as SLHI has learned over the years. Together with our community partners we continuously work toward greater levels of influence on the health and resiliency of Arizona’s diverse communities.
